licensed practical Nurse

An LPN works under the supervision of doctors and RNs, performing duties such as taking vital signs, collecting samples, administering medication, ensuring patient comfort, and reporting the status of their patients to the nurses. An LPN provides quality care to patients and performs technical skilled care in patient’s home in compliance with nursing standards, federal, state, local government regulations and company clinical policies.

  • Provides direct and individualized nursing care to assigned patients based on nursing standards and under the supervision of the Director of Nursing and/or a Registered Nurse designee.
  • Implements plan of care formulated by physician and RN.
  • Ensures quality and safe delivery of home health services to patients/clients and families/caregivers.
  • Participates in coordination of home health services with RN and clinical management.
  • Uses equipment and supplies effectively and efficiently.
  • Discusses health care with patient.
  • Provides basic medical and nursing care such as checking blood pressure and inserting catheters.
  • Ensures the comfort of patients by helping them bathe or dress.
  • Reports status of patients to physicians and RNs
  • Ensures quality and safe delivery of home health services to patients/clients and families/caregivers.
  • Provides accurate and timely documentation consistent with plan of care.
  • Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care.
  • Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.
  • At least one year of nursing experience.
  • Community/home health experience preferred.

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

OTHER SKILLS AND ABILITIES

  • Ability to communicate effectively with patient/client, family members, physicians and clinical management, and staff.

REASONING ABILITY

  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

The employee frequently is required to stand; walk; reach with hands and arms; and stoop, kneel, crouch, or crawl. The employee is occasionally required to sit. The employee must occasionally lift and/or move over 100 pounds. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
