If a patient has ongoing clinical needs due to acute or chronic illness, injury or disability, our care team will work closely with the medical team and family to provide this level of specialized care.

Our care regimen:

  • Assesses, provides, and monitors patients who require frequent adjustments to treatment regimens, and patients with medical conditions that require recurrent assessments and care plan changes.
  • Protects, manages and optimizes the health and outcome of the patient — generally with complex health issues — and well-being over time, mitigates the decline in quality of life and addresses adverse or preventable health triggers
  • Often involves complex advanced nursing care to prevent medical complications that may result in facility placement.
  • Offers companionship to calm agitation and anxiety, monitor safety to reduce risks of falls
  • Attends to daily needs and offers compassionate companionship.
  • Provides families much-needed peace of mind with quality care that mitigates the decline in quality of life and restores some independence.

Hourly Care
Skilled one-on-one care tailored to assist patients to facilitate recovery or comfort at home rather than being placed at a care facility.  Hourly care services helps patients maintain their daily routines and their independence.

Live-In Care
With live-in care, our patient can remain as independent as possible for as long as possible with a caregiver in their home twenty-four hours a day. Caregivers provide supervision, develop a deep understanding of care needs, and can create a real relationship with the patients they assist. 

24-Hour Care
24-hour care allows adults or seniors to remain at home while receiving the clinical and personal care they may need. This level of service encompasses patients that need in-home monitoring, those who require adjustments to treatment regimens often, and patients with medical conditions that require frequent assessments and care plan changes.

Hourly care may be helpful for those demonstrating some memory loss, forgetfulness and confusion (time lapse, bill payments, mail piling up, etc.); those needing help with balance or having trouble getting to the bathroom in time; and those with a general decline in mobility.

This level of care is also helpful for patients who need palliative care for extra support, comfort, and engagement while living with a long-term condition.

Round-the-clock care typically involves four to five caregivers who work eight- or twelve-hour shifts, are awake at all times, and provide supervision at all hours.

Clinical CARE Services

Transitioning from a hospital/rehab with a physical disability, or chronic condition.

Patients needing additional support or continuous assistance during day, at night, or round-the-clock care.

Administering medication including injections, inhalants, eye drops, oral, and topical medications.

Tracking oxygen levels, using heart monitors, or other advanced in-home equipment.

personal/home care Services

Assistance with household chores, preparing meals, shopping and other errands.

Help with personal care activities such as  bathing, oral hygiene and dressing.

Toileting & incontinence care.

Assistance with ambulation, walking and transferring from bed to wheelchair.

Supervision for those that wake up frequently in the middle of the night or patients needing palliative care.

Supervision and support for Alzheimer’s/ dementia patients who experience wandering or sundowning .
